True Colours Support Services provides individualized support to students attending public and private school systems, as well as adult day programs. True Colours Support Services works collaboratively with the school/program integrative and clinical team, associates corroborate an Individual Education Plan (IEP), an Individual Support Plan (ISP) for a student.
Hospitals & Healthcare Facilities
True Colours Support Services supports patients with complex needs throughout their stay in hospitals and healthcare facilities. True Colours Support Services works collaboratively with healthcare practitioners and other personnel. True Colours Support Services Associates provides support for the health and mental health needs of the individual but also any individuals that presents challenging behaviours.
Family Services; youth and adults
True Colours Support Services are delivered in the natural environment of the individual, whether it be a community program or a family home. Goals are established during the intake process and attuned throughout our involvement as needed.
Crisis Management
True Colours Support Services responds to all community and family crisis situations with immediacy. We are available 24/7/365. True Colours Support Services is prepared to support our community members with accessible service coordination and an extensive team of Associates skilled to safely and effectively support during crisis situations.
PROGRAMMING/Skill Development
True Colours Support Services offers community integration supports which allows Associates to work with the youth in their community after creating meaningful and measurable goals set by the probation officer, the youth and their family and other involved community services. Each goal includes a plan for achievement and regular review dates are provided. True Colours Support Services provides comprehensive behavioural support plans, safety plans and assessments. Youth Justice
True Colours Support Services support individuals between the ages of 12 and 17 who have gotten into trouble with the law. Our support can be provided during weekends, evenings, free time, and unstructured family time. Our Services can include:
- Preparing for and returning home following custody
- Living at home under conditions
- Attending school or work
- Family tension and conflict
- Developing positive peer relations and coping with peer pressure
- Managing independent or semi-independent living
- Assisting youth in receiving specific treatment e.g. drug and alcohol treatment
- Associate assistance and coverage for youth justice custody and detention facilities
