The RECLAIM programme moves from stabilization, to connection, to capability, to successful community transition.
ORIENTATION & STABILIZATION
Emphasis is placed on welcome, assessment, and ensuring physical and emotional safety. Staff establish trust, orient the youth to the home, and develop an initial care plan shaped by the youth's own voice.
PURPOSE
Stability is the ground on which every other part of the work stands. Nothing meaningful can be built until a young person feels safe and seen in the space they are in.
RELATIONSHIPS & REPRESENTATION
Focus shifts to relationships, identity, and cultural grounding. The youth is surrounded by adults who share or understand their lived experience, and community connections are intentionally cultivated.
PURPOSE
Healing happens in a relationship. This is where a young person begins to see themselves reflected in the adults around them and to recognise belonging as something available to them.
LIFE SKILLS & ACCOUNTABILITY
Attention turns to life skills, routines, and personal responsibility. Learning is structured, hands on, and paced to each youth, with goals set collaboratively and revisited together over time.
PURPOSE
Capability is built through practice and steady expectation. This phase gives young people the everyday competence and sense of ownership they will carry forward into independent life.
TRANSITION & FUTURE PLANNING
Planning becomes explicit. Youth, staff, and partners work together to design the transition, whether to family, independent living, or next placement, and to activate a support network before departure.
PURPOSE
A thoughtful release protects the gains of every earlier phase. The intent is that no young person leaves without a plan they understand, people they can call on, and a door that stays open.
